The Great Yarmouth and Waveney Older People's Community Service offer assessment and support in the community to adults experiencing memory issues and to older adults with a history of severe mental health problems combined with complex physical health frailties.

What this service offers
The team can work alongside other teams and organisations to provide interventions to aid recovery.
People who use this service will be living with the following conditions:
- Alzheimer's disease
- Dementia
- Fronto-temporal
- Lewy body
- Vascular
- Other related memory conditions.
They may also have:
- Anxiety
- Behavioural/Psychological symptoms of dementia (agitation, aggression)
- Depression
- Obsessional thoughts
- Personality changes
- Psychosis - hallucinations, delusions (perceptual issues / false beliefs)
- Suicidal thoughts.
Once somebody has been referred to the Great Yarmouth and Waveney Older People's Community Service a clinician may initially contact them by telephone to confirm:
- They would like to be assessed
- Whether they could access our clinic service or need to be seen at home.
The clinician may also gather a little extra information that may help with their assessment and be able to prioritise the urgency.
The person will then, for memory treatment, receive an appointment in the post.
Once in the clinic, they will see a specialist clinician, who will take the history of their concerns and do a test of their memory. The person may or may not be given a diagnosis at this time; sometimes more information needs to be gathered and further assessments undertaken.
Our team like people to have a loved one, close friend or relation with them to help with information and history details.
If somebody is referred for other mental health issues, the relevant clinician will contact them, often by phone, to arrange an appointment. Often this takes place in the home, but we do have Nurse Prescribers in the team who will be looking at offering some clinic appointments.
Access this service
Access to the Great Yarmouth and Waveney Older People's Community Service is made by referral by a GP or other registered health professionals.
If somebody has concerns about their memory or mental health, they should visit their doctor who will carry out some initial tests, bloods and ECG (electro-cardiograph), if necessary, and will then refer them to the appropriate service via the Access and Assessment Team (0300 123 1882).
Visiting and onsite facilities
Outpatient clinics are held for all adults with memory issues and 0lder people over 75 with enduring mental health problems.

Parking for visitors is available on site, this includes designated disabled parking spaces.
The site has full disabled access and toilet facilities.
How this service support carers including friends and family
The Great Yarmouth and Waveney Older People's Community Service offers assessment, support, advice, education, psychological therapies, signposting and guidance on an individual or group basis to carers where appropriate.
The team can offer either clinic or community visits.
Urgent help
If you are a service user and are in crisis and need support urgently you can contact your service on the number which will have been provided.
Call 111 and select the mental health option to speak to our 24 hour mental health crisis line.
If you are with someone who has attempted suicide, call 999 and stay with them until the ambulance arrives.
If anyone is at serious risk of harm, call 999 and ask for the police.
