Rehabilitation in complex psychosis

  • The team are a community mental health team in Norwich, supporting people from North Norfolk, Norwich and South Norfolk. The team aims to provide intensive, multi-disciplinary support to a defined cohort of 90 people with treatment resistant psychosis and to enable a meaningful personal and social recovery. Working proactively to prevent crises, it is the objective of the team to reduce the need for urgent and emergency care and acute inpatient admissions, minimise lengths of stay in hospital and reduce the use of out of area placements.

What this service offers

The Team will deliver mental health rehabilitation for adults with complex psychosis: a primary diagnosis of a psychotic illness (this includes schizophrenia, bipolar affective disorder, psychotic depression, delusional disorders, and schizoaffective disorder) with severe and treatment-resistant symptoms of psychosis and functional impairment. The service has three pathways*:

  1. Advice and guidance where specialist advice from rehabilitation professionals could enhance care already provided by supported accommodation or a CMHT.
  2. Specific support Intervention for example where a specific intervention is indicated, or where a service user requires enhanced wrap-around support around discharge to a supported accommodation provider, for a time limited period.
  3. Full team support and case management.

*The service will maintain links with the mother & Baby Unit (MBU) and will adhere to their policies and procedures in the care of women in the perinatal period (pregnancy and 12 months post-partum).

Access this service

This team covers service users from North Norfolk, Norwich and South Norfolk GP Primary Care Networks. Service users may be residing in mental health hospitals, supported living, residential care or independent living environments. The service is designed to follow the service user wherever possible regardless of where they are living.

Visiting and onsite facilities

The team makes the majority of contacts with the person in their home or community settings other than where the team is based. Free parking is available at the Hellesdon site.

How this service support carers including friends and family

Families and carers of those under the care of the team will be offered support and, with the consent of the service user, the opportunity to become more involved in discussions and decisions about their care, treatment and discharge planning. This includes:

  • A carers pack with guidance and support
  • Carers assessments (arranged upon request)
  • Specialist systemic therapy where appropriate.
