The role of the Home Treatment Team (HTT) - West
- Intensive support in your own home. Initially this will consist of a three day assessment to identify your needs and the support / treatment you require. The nature of this support will be discussed with you and the people who support you.
- An alternative to hospital.
What you can expect from the HTT
- Understanding of your current mental health issues.
- Prompt treatment and support, focused on recovery.
- Referral on to other agencies and mental health services, as agreed with you.
- Visits tailored to your needs, more than once a day, if required.
- Carer involvement and support with care plans and signposting to further community support for carers.
- 7-days-a-week input, including access to 24 hour advice – (see Contact us).
- Access to admission to a psychiatric ward where risk and presentation indicate Home Treatment is not appropriate, and support upon discharge if needed.
- Although the same member of staff may not attend every visit, all staff will be familiar with your situation.
- Review by a doctor when necessary.
- The HTT does not provide phone support for people not under their current care.
Your welfare
If you have been referred or are under the care of the HTT it is essential that we have an agreed plan, with up to date phone / carer details should we need to contact you.
If we are unable to make contact we may ultimately request assistance from the police, and on occasion (if we are concerned) the police may attempt to access your property.
