The Waveney Older People's Community Service offers assessment and support in the community to those experiencing memory issues and to older adults with a history of severe mental health problems combined with complex physical health frailties. What this service offers
This service offers assessments for both memory and functional mental health problems for Older Persons. This takes place in assessment clinics or in the home environment (if there is a clinical need).
Consultant Psychiatrists, experienced non-medical prescribers and mental health nurses deliver the assessments. This enables diagnosis and treatment plans where appropriate.
Where further input is not required the team can offer advice. This could include signposting to other relevant agencies.
Our community team holds regular meetings to discuss service user’s cases. This provides an opportunity for review with the consultant and other relevant team members. This ensures we provide appropriate support and advice. Along with guidance and signposting to other available services.
The team often works alongside other teams and organisations to provide interventions to aid recovery.
People who use this service will have one or more of the following conditions:
- Alzheimer's disease
- Dementia
- Fronto-temporal
- Lewy body
- Vascular
- Other related memory conditions.
- They may also have:
- Anxiety
- Behavioural/Psychological symptoms of dementia (agitation, aggression)
- Depression
- Obsessional thoughts
- Personality changes
- Psychosis - hallucinations, delusions (perceptual issues / false beliefs)
- Suicidal thoughts.
Once somebody has been referred to us, we will make contact with you by phone to talk with you about your needs and send out a letter confirming our contact, this also has lots of helpful telephone numbers.
Access this service
Access to the Older People’s Service Lowestoft and Waveney is by GP referral only.
People should visit their GP to discuss their concerns, who can then arrange for appropriate investigations, such as blood tests and ECG (electro-cardiograph) to rule out any physical changes.
If appropriate they will make a referral to this service. This is through our single point of advice line on 0300 123 1882, based at Northgate Hospital in Great Yarmouth. This service is open Monday to Friday, 9:00 to 17:00.

Our service provides full disabled access and toilet facilities.
A vending machine selling food and drinks is available.
How this service support carers including friends and family
Our team offers support, signposting advice, and education to carers. We can direct you to community services that can help you fill in forms for attendance allowance and to social services for assessment and support.
Urgent help
If you are a service user and are in crisis and need support urgently you can contact your service on the number which will have been provided.
Call 111 and select the mental health option to speak to our 24 hour mental health crisis line.
If you are with someone who has attempted suicide, call 999 and stay with them until the ambulance arrives.
If anyone is at serious risk of harm, call 999 and ask for the police.
